Growth of a thrombosed giant vertebral artery aneurysm after parent artery occlusion.

چکیده
Parent artery ligation is an accepted treatment for intracranial aneurysms that are not amenable to a direct surgical approach. Parent artery ligation is one of the oldest effective surgical means of aneurysm therapy (the "Hunterian principle" (1 ]). Recently it has become possible to perform parent artery occlusion of the carotid or vertebral artery percutaneously by using detachable balloon technology [2-5]. We report a case of a patient with a vertebral artery giant aneurysm, which was treated by parent artery occlusion of the vertebral artery, resulting in thrombosis of the aneurysm. In spite of thrombosis of the aneurysm and the parent artery, the aneurysm continued to grow. This case suggests that giant aneurysm growth is not always dependent on pulsatile blood flow as a driving force, and that recurrent mural hemorrhage into the highly vascularized aneurysm wall is a more likely mechanism. Giant aneurysms can be unstable lesions even when they (and their parent arteries) are thrombosed. Continued surveillance with CT andfor MR imaging is prudent following thrombosis of giant aneurysms by parent artery occlusion.

Giant thrombosed vertebral artery aneurysm treated by extracranial-intracranial bypass and aneurysmectomy--case report.
A 42-year-old male presented with a giant thrombosed aneurysm of the left vertebral artery. The aneurysm was resected through a combined subtemporal and suboccipital approach after a saphenous vein bypass graft was placed between the right external carotid artery and the posterior cerebral artery. Intraoperative measurements of the blood flow volume and pressure demonstrated good blood flow in ...
متن کاملGiant LV Thrombose and Thrombosed Coronary Aneurysm in an Infant with Kawasaki Disease
Background Kawasaki disease (KD) also known as mucocutaneous lymph node syndrome is the febrile and self-limited vasculitis that occurs in children of all ages, especially younger than five years of age. Here, we describe a case of Kawasaki disease with coronary artery aneurysm and a big clot in LV with a diameter of 1.5 cm. <stron...

traditional methods of treatment for ruptured dissecting aneurysm of vertebral artery (rVADA). Proximal artery occlusion may be effective in the acute period, but is not enough to eradicate the risk of rebleeding (1). Mass effect usually is not a concern for a case of well occluded rVADA. However, we experienced a growing thrombosed aneurysm after rVADA occlusion, and conventional angiography d...
متن کاملSpontaneous thrombosis in giant intracranial aneurysms.
Twelve patients in a series of 22 with giant intracranial aneurysms demonstrated neuroradiological features of partial or total spontaneous intra-aneurysmal thrombosis. The presence of this intra-aneurysmal clot significantly altered the computed tomographic appearance of the giant aneurysm. Massive intra-aneurysmal thrombosis did not protect against subarachnoid haemorrhage and the likelihood ...
متن کاملNeurological Deterioration after Decompressive Suboccipital Craniectomy in a Patient with a Brainstem-compressing Thrombosed Giant Aneurysm of the Vertebral Artery
We experienced a case of neurological deterioration after decompressive suboccipital craniectomy (DSC) in a patient with a brainstem-compressing thrombosed giant aneurysm of the vertebral artery (VA). A 60-year-old male harboring a thrombosed giant aneurysm (about 4 cm) of the right vertebral artery presented with quadriparesis. We treated the aneurysm by endovascular coil trapping of the right...
